General electrotherapy covers the three most widely used physiotherapy modalities: interferential therapy (IFT), therapeutic ultrasound, and combination therapy, which applies both at once. All three use energy, electrical current or sound waves, to reduce pain, calm muscle guarding and support the body's own healing process in an injured area.
They are not new and they are not dramatic. They are the well-understood, dependable end of the electrotherapy range, available in most physiotherapy clinics. What varies between clinics is not the machine but the judgement behind it: whether it is being used for a reason, at the right stage, as part of a plan.
Electrotherapy is an adjunct. It makes movement and hands-on treatment possible sooner by taking the edge off pain and muscle guarding, and that is a genuinely useful thing when pain is stopping you sleeping or stopping you moving. What it does not do is fix the underlying problem on its own. A course of electrotherapy with no assessment, no hands-on treatment and no exercise is not physiotherapy, and we do not offer it that way.
IFT passes two medium-frequency electrical currents through the skin from opposite directions. Where they cross, deep within the tissue, they combine to produce a low-frequency effect at the site of the problem, reaching deeper than surface stimulation can, with far less discomfort at the skin.
It is used mainly for pain relief, to reduce swelling, and to settle muscle spasm around an irritated joint. Most patients find it the most comfortable modality in the range: you feel a light tingling or prickling that fades as you get used to it, and it should never feel sharp or burning.
Commonly used for: lower back pain, neck and shoulder pain, knee pain, post-injury swelling, and muscle spasm.
Therapeutic ultrasound delivers high-frequency sound waves into soft tissue through a handheld head and a layer of gel. Depending on the settings used, it produces either a gentle deep warming effect or a non-thermal effect intended to support tissue repair. You will feel the movement of the head and possibly mild warmth, but nothing more.
It is used on soft tissue rather than on bone or joints: muscle strains, tendon problems, and areas of thickened or restricted tissue. Its evidence base is moderate rather than strong, which is why we use it where it has a clear rationale and alongside manual therapy and exercise, not as a standalone course.
Commonly used for: muscle strains, tendon irritation, soft tissue tightness, and localised areas of restriction.
Combination therapy applies ultrasound and electrical stimulation through the same treatment head at the same time. The practical advantage is precision: the combined current makes a tender, tightly localised point in a muscle noticeably easier to find and treat accurately.
It is most useful where pain is coming from a specific, identifiable trigger point rather than a diffuse area: a knot in the upper trapezius driving a headache, or a tight band in the calf or gluteal muscles. Sessions are short and targeted, and it is usually combined with manual release of the same area in the same visit.
Commonly used for: trigger points, localised muscle knots, and stubborn areas that have not responded to hands-on work alone.

Electrotherapy is almost always used within a physiotherapy appointment rather than booked on its own. Your therapist decides which modality suits the stage you are at and why, and explains it before starting.
The area is exposed and either electrode pads are placed on the skin for IFT, or gel and a treatment head are used for ultrasound. You stay still and comfortable throughout. IFT feels like a light tingling; ultrasound feels like gentle movement and possibly warmth. Neither should hurt. Tell your therapist immediately if anything feels sharp, burning or uncomfortable, as the intensity can be adjusted instantly.
Hands-on treatment and your exercise programme follow in the same visit.
How long each session takes and how many you need is decided at your assessment, not before it. It depends on what is causing the problem and how long you have had it. You will be given an estimate after the first visit rather than asked to buy a package in advance.
None of these automatically rules out treatment. They change which modality is appropriate and where it can be applied, which is exactly why the assessment comes first. Your therapist will ask, but please mention anything on this list even if you are not asked.

No. IFT produces a light tingling or prickling sensation that most people find comfortable and stop noticing after a minute or two. Ultrasound feels like gentle movement and sometimes mild warmth. Nothing should feel sharp, burning or painful. Say so immediately if it does, and the intensity will be adjusted.
Electrotherapy is not prescribed as a course of its own. It is used within your physiotherapy sessions for as long as it is helping, and dropped once it is not. If a clinic offers you a fixed package of electrotherapy sessions without an assessment, that is worth questioning.
Neither replaces the other. Electrotherapy is useful for reducing pain and muscle guarding so that hands-on treatment and exercise can be done more effectively and sooner. The lasting change comes from the movement, strength and load work.
IFT uses electrical current and is mainly for pain, swelling and muscle spasm, applied over a broader area through pads. Ultrasound uses sound waves on soft tissue and is applied to a smaller, specific area with a handheld head. Your therapist chooses based on what is driving your symptoms.
Tell your physiotherapist before treatment. A pacemaker or implanted electrical device rules out electrical modalities, and metal implants change where ultrasound can be applied. Neither means physiotherapy is unsuitable: it means a different approach is chosen.
